1What is Rh Immune Globulin (RhIG)?
- a pharmaceutical drug that contains anti-D ab
- given to Rh negative mothers who are KNOWN to be negative for allo anti-D AND who have been potentially exposed to Rh+ RBCs (Rh+ baby)
2How is RhIG administered?
intramuscular injection via a pre-measured syringe (1mL in volume) with attached needle
3How many doses are given in RhIG?
1. Micro Dose
2. Full Dose
4What are the indications in using the MICRO dose RhIG?
after abortion, miscarriage, or termination of ectopic pregnancy
5What is the administration for MICRO dose RhIG?
at 12 weeks gestation (antepartum)
6How much RhIG is in the MICRO dose?
- micro dose = 50 µg of anti-D
- protects against 2.5 mls of Rh+ RBCs or 5 mls of Rh+ WB
7What are the indications for the FULL dose RhIG?
after amniocentesis / cordocentesis, abdominal trauma during pregnancy, antepartum, antepartum hemorrhage, or postpartum
8When is the FULL dose RhIG administered?
- following an abortion or miscarriage after 12 weeks gestation
- ≥ 28 weeks gestation (antepartum)
- Within 72 hours postpartum:
❖ When infant is Rh positive OR the fetus Rh is unknown, e.g., fetal demise / stillborn, miscarriage, etc.
❖ The mother has not developed an allo-anti-D
9What is the FULL dose of RhIG?
Full dose = 300 µg of anti-D; protects against 15 ml of Rh+ RBCs or 30 mls of Rh+ WB
10What is the calculation for how much RhIG to give?
Fetal cells vol (from KB Test result) / 30 = # syringes given
❑ When calculating the # of RhIG vials, include 1 decimal place, i.e., X.X
❑ Rounding up or down:
➢ If the decimal number is < 0.5 - round down the calculated value, e.g., 2.2 = 2
➢ If the decimal number is ≥ 0.5 - round up the calculated value, e.g., 2.7 = 3
❑ Because this is a "calculation", it is only an "estimate". Therefore, always add 1 to your calculations to ensure adequate dose, e.g., calculated dose is 2 + 1 = 3; calculated dose is 3 + 1 = 4
11What is the Rh Factor Sensitization Prevention?
12Besides the RhIG intramuscular syringe, what other treatments are there?
1. IV RhIG given when there is accidential transfusion of Rh+ given in large blood volume
2. Treatment of ITP
13When is IV RhIG administered instead of intramuscular RhIG?
- when blood volume is larger than 30 mLs
- ex. WinRho - pharmaceutical drug
- BB can calculate amount needed based on quanitity of Rh+ RBCs transfused
14What is ITP?
- Immune thrombocytopenia purpura (ITP)
- a rare autoimmune disorder that occurs when the body's immune system
destroys platelets, which are blood cells that help blood clot
- results in petechiae
15What is the treatment of ITP?
