1What are the origins and course of PDOs?
- childhood onset (born with, experiences change person; ex = reinforcement for lying)
- chronic
2What are the 3 clusters of PDOs?
- cluster A: Odd eccentric PDOs (flat affect, strange cognition, emotional problems) 3/4
- cluster B: Dramatic - emotional, interpersonal PDOs (lots of emotions, maladaptive cognitions/ thoughts, maladaptive relationships, impulse control) 4/4
- cluster C: Anxious-fearful PDOs (high anxiety, relationship problems, maladaptive cognitions/thoughts) 3/4
3What PDOs are in Cluster A?
- paranoid (distrust/ sus of others such that their motives are interpreted as intentional)
- schizoid (pattern of detachment from soc relationships and restricted range of emotions in interpersonal settings)
- schizotypal (pattern of soc/ intp defecits, acute discomfort in relationships, cognitive and perceptual distortions, eccentricness in behaviors)
4What PDOs are in Cluster B?
- antisocial (disregard/ violating rights of others)
- borderline (instability of intp relationships, self-image, affects, and control over impulses)
- histrionic (excessive emotion and attention seeking)\
- narcissistic (pattern of grandiosity (in fantasy or behavior), need for admiration, lack of empathy)
5What PDOs are in Cluster C?
- dependent (excessive need to be taken care of, leads to submissive/ clinging behavior and fears of separation)
- avoidant (pattern of social inhibition, feelings of inadequacy, hypersensitivity to neg evaluation)
- OCPD (orderliness, perfectionism, mental and interpersonal control, at the expense of flexibility, openness, and efficiency)
6What cluster tends to have the highest comorbidity?
cluster B (4/4 maladaptive/ deficits so naturally higher)
7What defines a personality disorder?
- pervasive and inflexible
- causes distress/ chronic impairment
- high comorbidity w/ other PDOs
- poorer prognosis (not easy to change, maladaptive)
8What are the Maladaptive behaviors/ Deficits present in PDOs?
At least 2 areas:
- cognition (think a certain way only)
- affect (loss of emotion)
- interpersonal functioning (relationships: develop, maintain)
- impulse control
9Can people with PDOs adapt and be flexible?
Yes, but this flexibility causes stress/ impairment
10What is the prevalence rate for all PDOs (generally speaking?)
10 % gen pop
11What is the prevalence rate of Paranoid PDO? (Cluster A)
3% of gen pop
12PPD: M or F? How does the presentation differ by gender?
M > F
- M: tends to be explosive, angry, in your face
- F: suspicion, trauma
13PPD: What is the cognitive pattern of distrust and suspiciousness?
- assume exploitation, harm, deception
- shocked by loyalty
- interprets mistakes as slights
14PPD: What are the behavioral expressions?
- sarcastic, blaming
- legal battles
- slight level of delusion (grandiose; I'm better than them)
- prefer simple and straightforward (muddy = bad)
- seek similar others
15PPD: Are there high levels of stress involved?
yes. borders schizophrenia (paranoid)
16PPD: Treatment?
Really isn't a treatment option.
- other people need treatment
- don't seek treatment on own accord
17What is the prevalence rate of Schizoid PDO? (Cluster A)
0.8 - 1.5% of gen pop
18Schizoid PDO: M or F?
M > F
19Schizoid PDO: what are the 2 main symptoms?
- detachment (affective flattening)
- restricted affect
20Schizoid PDO: What are the characteristics of detachment?
- lack of desire for social intimacy
- indiff to soc situations/ cues
- indiff to criticism/ praise (no emotional toll on them)
- lack goals/ interests (no emotion)
21Schizoid PDO: What are the characteristics of restricted affect?
- lack of pleasure in relationships/ interactions
- lack of pleasure in sensory experiences (eating good food)
- lack of social reciprocity (shake hand?)
- lack of emotions w/ extreme experiences (very blunted)
- expression of anger is difficult
22Schizoid PDO: Childhood experiences?
- pathologically shy in childhood
- generally attributed to anxiety, in reality it's just lack of interest/ meaning)
23What is the prevalence rate for Schizotypal PDO? (Cluster A)
3.9% of gen pop
24Schizotypal PDO: M or F?
M > F (for all cluster A PDOs)
25Schizotypal PDO: How does it differentiate from schizophrenia?
- same distinguishing features, BUT schizotypal is aware of oddities, which causes depression sometimes
26Schizotypal PDO: What is the comorbidity rate with depression?
30 - 50%
27Schizotypal PDO: What marks the pervasive patterns of interpersonal deficits?
- acute discomfort in relationships
- cognitive and perceptual distortions
- eccentricities in behaviors (low social acceptance; sit alone at lunch)
28Schizotypal PDO: What are ways they are superstitious and/or paranormal in their behavior?
- reference delusions/ beliefs (random things have more meaning for no reason; specks on wall, still grounded in reality)
- comply w/ magic rituals (ex: #13 is cursed)
- perceptual alterations (experience things as "90%" or "110%"; more odd explanations for simple things)
29Schizotypal PDO: What are ways they are odd?
- speech (tangential)
- behaviors (strange gait; head bent at a weird angle, sit weirdly)
- mannerisms and dress (weather appropriate, just weird combos)
