1
Disease classification?
Botulism
2
CO?
Clostridium botulinum
3
Classification of CO? (structure, behavoir, what does it produce)
*gram positive rod
*strict anearobe
*non-lactose fermenting
*spore forming
*produces neurotoxins (A-G and H)
4
What is lethal dose (LD50) of Clostridium botulinum?
2-1000 ng/kg based on portal of entry
5
Name of specific neurotoxin?
Botulinum toxin (BTX)
6
How do BTX interact with cells?
*Botulism neurotoxins (BoNTs) bind to periphercal cholinergic nerve terminals (accepts acetylcholine)
*BoNTs enters cytosol of cell through endocytosis
*once in cell, light chain cleaves SNARE proteins (blocking release of neurotransmitters).
7
Why does cleavage of SNARE proteins matter?
SNARE proteins forma complex that enables synaptic fusion of vesicle containg neurotransmitters (Acetylcholine) to synaptic membrane
8
Reservoir?
Environment (soil)
9
Portal of entry?
*digestive tract or open wounds
10
Mode of Transmission?
*eating contaminated foods
*parenteral route (administering liquids through IV)
11
Incubation?
6 hrs-10 dys
12
Pathogenesis of Botulism?
*spores germinate and colonize food
*bacterium enters through intestinal mucosa or tissue
*secreted botulinum toxin enters circulatory system
*BoTX transported to motor neurons, it blocks release of neurotransmitters, results in paralysis
13
What systems does Botulism impact?
*gastrointestinal
*nervous system
14
Gastrointestional symptoms associated with Botulism?
*constipation
*nausea
*vomiting
*dry mouth
*chills
*thick-feeling tongue
15
Nervous system symptoms associated with Botulism?
*blurred vision
*dilated pupil
*dropping eyelids
*inability to swallow
*speech impairment
*urinary retention
*skeletal weakness
16
What botulism symptoms are common with infants?
*lethargy
*weak cry
17
Complications of botulism?
*parlysis
*respiratory failure
*mortality (5%)
*sequelae (pathological consequence resulting from previous disease or injury)
-muscle atrophy
-long lasting weakness
-shortness of breath
-secondary infection
18
Prevention of Botulism?
*proper canning procedures
*proper food handling storage
*boil canned foods for 10 min
*dispose contaminated foods
*don't consume suspect foods
19
Treatment for Botulism? Be specific for babies?
*Antitoxin (for Type A, B, E, and H)
-BabyBIG-human IgG for types A and B (infant botulism)
*remove gastrointestinal contents
*life support systems (ventilator)
