1
What is the anode heel effect?
• X-ray intensity is NOT uniform across the primary beam
• Intensity is HIGHER on the CATHODE side
• Intensity is LOWER on the ANODE side
• Creates a gradient of intensity from one end of the field to the other
2
Which side has higher intensity — cathode or anode?
• CATHODE side — HIGHER intensity
• ANODE side — LOWER intensity
3
How do you apply the heel effect clinically?
• Place THICKER/denser part of patient toward the CATHODE (more intensity needed)
• Place THINNER/less dense part toward the ANODE (less intensity needed)
• Compensates for variation in patient thickness
• Produces more uniformly exposed image
4
Give one body part example of correct heel effect orientation.
• AP SPINE: lower lumbar (thicker, denser) → CATHODE end
• Cervicalpper thoracic (thinner) → ANODE end
• OR: AP FEMUR — proximal hip (thicker) → CATHODE, knee (thinner) → ANODE
5
Explain WHY intensity is lower on the anode side.
• X-rays produced at the focal spot on the ANGLED anode surface
• X-rays emitted toward the anode direction must travel THROUGH more anode material
• More material = more attenuation (absorption)
• X-rays toward the cathode side travel through little or no anode material
• Result: anode-side photons are more attenuated → lower intensity
6
State three factors that INCREASE the magnitude of the heel effect
• Shorter SID — beam diverges more over a shorter distance, accentuating non-uniformity
• Larger field size — includes more of the peripheral beam where effect is greatest
• Steeper anode angle (smaller degrees) — greater path length difference on anode side
• Thicker anode material
7
What is the relationship between anode angle and the heel effect?
• Steeper angle (fewer degrees, e.g. 6°) → MORE pronounced heel effect
• Greater path length difference between anode-side and cathode-side X-rays
• BUT steeper angle also → SMALLER effective focal spot (line focus benefit)
• AND steeper angle → SMALLER usable field size
• Trade-off: anode angle (6–20°) must balance all three factors
8
A radiographer places the proximal femur (hip) toward the ANODE end for an AP femur. What happens?
• Proximal femur/hip = THICKER, denser region → needs MORE X-rays
• Anode side = LOWER intensity → INSUFFICIENT photons through the dense proximal femur
• Result: proximal femur UNDEREXPOSED — too dark, poor detail
• Knee end = THINNER → on cathode side = HIGHER intensity → OVEREXPOSED
• Non-uniform image: one end diagnostically inadequate
• May require repeat → doubles patient dose
9
Why is the heel effect less clinically significant in modern digital radiography?
• Digital detectors have WIDE DYNAMIC RANGE — capture broader range of exposures
• Digital post-processing (equalisation algorithms) adjusts brightness across the image
• Film: heel effect could push parts of the image outside the usable exposure range
• Digital: system adapts automatically
• However: extreme errors on thick body parts still cause visible non-uniformity
• Correct orientation still best practice regardless of detector type
