1
What pain assessment tools do we use if the patient cannot communicate and how often do we use them?
Behavioral pain scale (BPS)
Critical-Care pain observation tool (CPOT)

Assessed every 4 hours/very regularly
2
Pain cutoff for BPS
>5=pain
3
Pain cutoff for CPOT
>3=pain
4
Fentanyl considerations
Tolerated better than alternatives in patients with hemodynamic instability
Can accumulate in when used for long periods or with hepatic impairment
tachyphylaxis after prolonged use (tolerance)
Serotonin syndrome*
Most commonly used medication
5
Hydromorphone considerations
Accumulation in hepatic failure
Longer duration
6
Morphine considerations
Histamine mediated hypotension, avoid in hemodynamically unstable patients
Accumulation in renal and hepatic failure
Morphine metabolite accumulation can lead to additional side effects, especially in renal failure:
morphine-3-glucuronide: can cause seizures and agitation
morphine-6-glucuronide: can cause more sedation and respiratory depression
7
Methadone considerations
Not commonly used, long half life doesn't make it suitable for as needed use
QTC prolongation
accumulation in hepatic failure
8
Hydrocodone considerations
Not commonly used, but used when transitioning patient to oral agent
accumulation in renal and hepatic failure
9
Most commonly used adjunctive treatments for pain
Acetaminophen
Ketamine
Anticonvulsants for neuropathic pain

NSAIDs can be considered but only for short durations
10
Sedation-Agitation Scale (SAS) goal
3-4
11
Richmond Agitation-Sedation Scale (RASS) goal
0 to -2
12
What medications do we use to maintain light sedation
Propofol
Dexmedetomidine

Avoid benzos
13
Propofol considerations
No analgesia
Can cause respiratory depression
Can cause brayarrythmias and hypotension
High dose infusions >48hours can cause hypertryglyceridemia
More effective at reaching deeper sedation levels
Monitor for PRIS
14
What to monitor for propofol
Vitals
SCr, UOP (urine output), triglycerides, amylase/lipase
15
What is PRIS
Propofol infusion syndrome, discontinue immediately if there are signs/symptoms

Occurs at high doses when used for >48 hours
16
Who should propofol be avoided in?
acute liver failure or pancreatitis patients, hard to tell PRIS symptoms apart from their baseline
17
Dexmedetomidine considerations
Good for light sedation, not deep
May be analgesic
Can cause hypotension and bradycardia
Monitor for vitals and discontinue slowly due to dependence
18
What is the cutoff on the intensive care delirium screening checklist (ICDSC)
>4=dilirium
19
Quetiapine (seroquel) dosing for dilirium
50mg Q12H, with a max dose of 200mg Q12H, best evidence out of antipsychotics
20
Etomidate dose for RSI
0.3mg/kg IVP
21
Ketamine dose for RSI
1-2mg/kg IVP
22
Propofol dose for RSI
1-2mg/kg IVP
23
Succinylcholine dose for RSI
1-1.5mg/kg IVP
24
Rocuronium dose for RSI
1mg/kg IVP
