black/ blurry spots in the vision
associated with migraines
can be caused by glaucoma or nerve disorders
9
What is lacrimation?
tearing
irritants or obstruction in drainage of tears
10
What is myopia?
nearsighted, trouble seeing objects far away
globe is longer than normal and light rays focus in front of retina
11
What is hyperopia?
farsighted, trouble seeing up close
globe is shorter than normal and light rays would focus behind the retina
12
What is ptosis?
drooping of upper eyelid
13
Do cataracts and glaucoma cause pain?
No
14
What is purulent discharge?
thick and yellow
crusts form at night
15
What is conjunctival hemorrhage normal for?
blood thinners
16
Position for objective data
standing for vision screening
sitting with head at eye level
17
Equipment for objective data
Snellen eye chart
Handheld visual screener
Penlight
18
Far vision
Snellen Eye chart -> well lit and at eye level
20 feet from chart
hand person opaque card to shield one eye at a time during test -> so person can't memorize
leave glasses/contacts on
start with smallest line
move patient closer to chart until they can see
19
Vision documentation?
Top # distance of patient from the chart
OVER
Bottom # distance normal eye could have read the line
Patient/perfect
20
Near vision
Visual Screener (handheld) (use magazine if needed)
hold card 14 inches away = 20/20 on Snellen distance
test each eye separately
normal result is 14/14 bilaterally
should be read without hesitancy and without moving card closer or farther away
21
What is presbyopia?
decrease in power of accommodation with aging
suggested when person moves card farther away
22
Peripheral vision
Confrontation test
Distance 2 feet -> personal distance
Cover one eye (patient) examiner covers OPPOSITE eye -> to know when they should be seeing it
Use object and slowly advance from periphery in several directions
Repeat for opposite eye
Normal: peripheral vision intact
23
What is the goal for the peripheral vision test?
person sees object when examiner does
24
What does the corneal light reflex assess?
assess the parallel alignment of the eye axes by shining a light towards the person's eye
25
Corneal Light Reflex
Hirschberg Test -> checking light reflex with muscle function
Assess strength of eye muscles
Patient to stare straight ahead
Hold light 12 inches away
Note reflection of light on corneas
26
What is the goal of the corneal light reflex?
reflection same spot on each eye
27
What does asymmetry of light mean with the corneal light reflex test?
deterioration in alignment from eye muscle weakness or paralysis
28
What is esotropia?
eye turned inward
light will reflect on outer pupil
29
What is exotropia?
eye turned outward
light will reflect on inner eye
30
What is hypertropia?
eye turned upward
light will reflect on bottom of pupil
31
What is hypotropia?
eye turned downward
light will reflect on upper of pupil
32
Cover test **?? (PG 305?)
detects weakness in a particular eye
patient stares straight ahead
cover one eye at a time
note uncovered eye
eye should not be jumping after uncovering
33
What is the goal for the cover test?
response is a steady fixed gaze
34
Diagnostic Positions Test
examiner guides patient
object held at 12 inches
patient to follow examiner movement with eyes only, hold head still
go through 6 cardinal positions
35
What does the diagnostic positions test examine?
examines for muscle weakness during movements
36
What is nystagmus?
a fine, oscillating movement best seen around the iris
37
What does nystagmus occur with?
occurs with disease of semicircular canals in ears, paretic eye muscle, multiple sclerosis or brain lesions
38
What is the goal for diagnostic positions test?
parallel tracking with both eyes
39
Eyebrow Inspection
symmetrical, present bilaterally
don't makes assumptions and ask why
scaling with seborrhea
moves with expression changes (unless they've had botox)
40
Eyelids and lashes
normal overlap
eyelashes are evenly distributed
intact without redness
41
What is lid lag related to?
hyperthyroidism
42
Lacrimal Apparatus
no pain
normal tears, nose runs
pay attention to one eye tearing up more than the other
swelling visible bulge in outer part of upper lid
43
What is the normal sclera?
the white, tough, opaque, fibrous part of the eye
China white
44
What may African Americans have in their conjunctiva?
yellow fatty deposits
this is normal
45

What is scleral icterus?
even yellowing of the sclera extending up to the corner
indicating jaundice
50
Note the following with pupillary light reflex?
size/shape -> symmetrical
normally pupils appear round, regular and of equal size
51
Use your penlight for?
side to side comparison
person gazes at a distance
light shining in eye -> constrict and both eyes should be the same
52
What is direct light reflex?
constriction of pupil shining the light on
53
What is consensual light reflex?
simultaneous constriction of the other pupil not having light shined into it
54
Accommodation testing
